Cost of Application: Rs.1000/-
SC/ST candidates are extempted from payment of fees for the application forms subject to production of an attested Xerox copy of community certificate.

Mode of Payment (DD):
Demand Draft in favour of "The Secretary, Selection Committee, Chennai - 600 010" payable at Chennai from any Nationalised bank on or after 20.04.2008. The name and address of the candidate should be written in the reverse of the demand draft.
Application Forms:

• In Person: Application forms can be obtained with a written requisition to the Deans of the below Institutions duly enclosing a crossed Demand Draft from the following Colleges:
  > Madras Medical College, Chennai - 600 003
  > Madurai Medical College, Madurai - 625 020
  
• Through Website: Application Forms can be downloaded from the website www.tnhealth.org. and can be submitted along with demand draft before last date specified.

The completed application forms should reach "The Secretary, Selection Committee, No.162, Periyar E.V.R. High Road, Kilpauk, Chennai - 600 010" on or before 30.04.2008 by 05.00 PM.
